Thank you for choosing our care team for your medical care. If you are a new patient, 请 ask your primary care provider (PCP) or other physician to forward a referral with all pertinent medical records. If you do not have a PCP or referring doctor, 请 call our appointment line to talk with our team. We would be happy to work with you to get you scheduled with our team. 请参阅我们的 Referrals and Precertifications page for more information about referrals. A referral is not required to be seen in our practice, but it is important you check with your insurance provider if it is required to be covered under your plan.
Once we receive your information, we will contact you to schedule your appointment. We may recommend testing before your appointment, which we can help you coordinate. Some of our locations offer both telehealth and in-person appointments. The scheduling secretary will explain your options to you in detail depending on the location you choose.
